What this lesson teaches

  • I can explain how a shared responsibility approach helps people in need and the challenges it brings.

Syllabus K230, Issue 2, Guiding Question 3, Citizens and government responding to socio-economic diversity. A shared responsibility approach and its challenges

Make a guess

In Singapore, who do you think pays for a large hospital bill?

  1. The patient, insurance and the government share the cost between them.
  2. The patient, from their own savings in MediSave.
  3. The government, so the patient pays very little for the stay.
Show the answer

The patient, insurance and the government share the cost between them.

MediSave savings, MediShield Life insurance and subsidies share the cost. MediFund is a safety net for those still unable to pay.

In a shared responsibility approach, individuals, families and the government each contribute, with extra help for those who still cannot afford it.

Shared responsibility combines several ways of paying: government subsidies (help towards costs), personal savings, insurance and direct payments. Savings put aside a person's own money for future needs. Insurance pools regular payments, called premiums, from many people to cover big risks, such as large medical bills. Savings and insurance do different jobs.

Singapore's healthcare works like this. MediSave is savings. MediShield Life is insurance. MediFund is a safety net for those who still cannot pay.

Sharing spreads the cost and encourages people to prepare for the future. It does not mean everyone pays the same, or that the government steps back. Government may still be the biggest contributor. The system must allow for differences in income, needs and ability to pay.

Some people cannot save enough because of low pay, gaps in work, caring duties or high costs. Complicated applications and unclear information can also block access. A safety net gives extra help when people still cannot meet essential needs. Help must be enough and easy to get. Having several schemes does not prove every gap is covered. Rising costs put pressure on both families and government.

Real examples to use

MediSave, MediShield Life and MediFund

Singaporeans pay medical bills using their own savings (MediSave) and insurance (MediShield Life), with government subsidies. MediFund is a safety net for those who still cannot pay.

Worked example: A hospital bill

A worker's hospital bill is reduced by government subsidies and MediShield Life. Her MediSave covers part of the rest. Because her income is irregular, she still cannot pay the balance, so she applies to MediFund.

  1. Subsidies and insurance reduce the bill.
  2. Her own savings cover some of the rest, but irregular income made it hard to save.
  3. The safety net helps with what she cannot afford. To judge the system, check whether she actually gets enough help in time: a safety net existing is not proof that it works.

Watch out for this

Shared responsibility means people must pay for everything themselves, or that anyone can save enough if they try.

The government still pays a large share, and safety nets help those who cannot save enough.

Check your understanding

What is the role of MediFund in Singapore's shared healthcare system?

  1. A national insurance plan that helps people pay for large hospital bills.
  2. A personal savings account that each worker builds up for healthcare.
  3. A safety net for patients who still cannot pay after subsidies and savings.
Show the answer

A safety net for patients who still cannot pay after subsidies and savings.

Right. It catches those the other parts do not fully cover.
